(1) a credit card applicatoin for a certification of enrollment under sections 1321.51 to 1321.60 associated with the Revised Code shall include an undertaking by the applicant to adhere to those parts. The program will be on paper, under oath, plus in the proper execution recommended by the unit of banking institutions, and shall include any information that the unit may necessitate.
